If you have set the reload interval to 0 then AFAIK SC will not
automatically add your music from Itunes.  What it does (and it's not
something I know a huge amount about, so happy to be corrected) is
check at the start of every reload interval whether there have been any
changes in Itunes and if so, reloads the info into SC.

The constant recanning suggests that something is not quite right with
your setup however.  If you are only using Itunes, jaffacake's advice
is spot on - also check you have the right .xml file input. iTunes does
and can create more than one .xml file - you need the one where the file
stamp changes when you make changes in iTunes.  

Are your music files and/or your .xml file on a different drive to SC -
a network attached drive at all?
